Từ 80 năm trước người ta đã thấy tình trạng dinh dưỡng kém của người mẹ ảnh hưởng xấu đến sự phát triển của thai nhi và trẻ sơ sinh. Những phát hiện từ sớm đó mở ra nhiều nghiên cứu làm cơ sở cho các chương trình y tế công cộng nhằm cải thiện tình trạng sức khỏe thai nhi và trẻ sơ sinh. Ngày nay, phụ nữ có thai trên khắp thế giới được khuyên ăn đủ chất và bổ sung thêm đa vi chất, ít nhất là sắt và axit folic. Những kết quả nghiên cứu dưới đây đưa ra cơ sở của khuyến nghị đó. Chậm phát triển và trưởng thành ở trẻ em chủ yếu xuất hiện ngay từ khi trong bụng mẹ và trong 3 năm đầu đời. Người mẹ nhỏ bé và chế độ ăn uống thiếu thốn trong khi mang thai là những nguyên nhân chính dẫn đến chậm phát triển bào thai. Chậm phát triển ở trẻ nhỏ có quan hệ chặt chẽ tới tiêu chảy, bú mẹ không đủ và thiếu thực phẩm ăn bổ sung, ăn dặm. Chậm phát triển từ trong bào thai và trong những năm đầu đời dự báo tổn thương chức năng của đứa trẻ (chậm phát triển cả tinh thần và vận động) cũng như khi trưởng thành (giảm khả năng sinh sản, trí tuệ và năng lực làm việc) Dinh dưỡng kém từ trong bào thai làm gia tăng nguy cơ mắc béo phì và bệnh không lây sau này (đái tháo đường type 2 và bệnh tim mạch), hiện tượng được gọi là ‘lập trình bào thai, fetal program-ming’. Măc dù từ năm 1990 đến nay, chất lượng và số lượng thực phẩm cung cấp ở Việt Nam đã tăng lên nhiều nhưng tình trạng còi cọc vẫn phổ biến. Việt Nam được đánh giá tốt trong thực hiện các chương trình y tế công cộng nâng cao tình trạng dinh dưỡng cho bà mẹ và trẻ em, vì vậy sẽ góp phần vào giảm tỷ lệ thấp còi.

The devastating effect of poor maternal nutrition on fetal and infant growth was recognized 80 years ago. Those early observations lead to numerous research studies that form the basis for public health programs to improve pregnancy outcomes and health of the child. Today, pregnant women, worldwide, are advised to eat a quality diet and to take a micronutrient supplement that provides at least iron and folate. The following research findings provide the basis for that recommendation. Growth failure and manurational delays in the child occur mainly in utero and the first 3 years of life. Small maternal size and inadequate dietary intakes during pregnancy are the major causes of fetal growth retardation. Growth failure in early childhood is intimately associated with diarrheal disease, lack of breastfeeding, and insufficient sources of weaning foods. Growth failure in utero and early in life predicts functional impairment in the child (delayed motor and mental development) as well as in the adult (reduced reproductive and intellectual performance and diminished work capacity). In utero exposures to poor nutrition increase the risk of obesity and non-communicable disease (type 2 diabetes and cardiovascular disease) later in life, a phenomenon called ‘fetal programming’. Although the quality and quanlity of the food supply in Vietnam has increased dramatically since 1990, stunting is still prevalent. Vietnam is well posed to implement public health programs to improve maternal and child nutrition and, thereby, reduce the prevalence of stunting.
