Những kết luận mới của luận án

Nghiên cứu tư vấn chế độ ăn, thực phẩm có bổ sung đường isomalt (như bánh Huralight, bột dinh dưỡng Quasurelight) phối hợp với luyện tập làm giảm nồng độ glucose máu, cải thiện tình trạng mỡ máu và nhân trắc ở người tiền ĐTĐ góp phần làm giảm các yếu tố nguy cơ và làm chậm sự tiến triển của bệnh ở người tiền đái tháo đường. Kết quả nghiên cứu cho thấy:

  • Ở những người tiền ĐTĐ, nồng độ glucose máu ở nhóm can thiệp giảm 65,3% sau 4 tháng can thiệp (p<0,01). Tỷ lệ tiến triển thành ĐTĐ2 ở nhóm can thiệp (13,6%) thấp hơn nhóm chứng(27,9%), với p<0.05
  • Nhóm can thiệp tỷ lệ tiến triển từ tiền ĐTĐ thành ĐTĐ2 giảm 14,3% so với nhóm đối chứng (p<0.05)
  • Nồng độ cholesterol, LDL giảm nhóm can thiệp (p<0.01). Ngược lại tỷ lệ này tăng ở nhóm đối chứng.
  • Tỷ lệ ở nhóm can thiệp  giảm nhiều hơn so với nhóm đối chứng từ 61,5% (n=32) xuống 28,8% (n=15), p<0.05. HSHQ=24,3%.
  • Sau can thiệp lượng gạo và dầu mỡ tiêu thụ tính theo đầu người đã giảm đi nhiều, sự khác biệt có ý nghĩa thống kê, với p<0.01. Các loại rau xanh, lạc vừng, quả chín tăng lên đáng kể ở nhóm can thiệp, lượng chất mặn và thịt các loại, dầu mỡ giảm  đáng kể có ý nghĩa thống kê, với p <0.05. Các loại đạm như cá, hải sản tăng lên ở nhóm can thiệp, có ý nghĩa thống kê với p<0.01.
  • Kiến thức, thực hành về luyện tập phòng, chống ĐTĐ của người tiền ĐTĐ tại nhóm can thiệp được cải thiện đáng kể so với nhóm đối chứng.

Introduction: Early intervention and avoidance or delay of progression to Type 2 diabetes is of enormous benefit to patients in terms of increasing life expectancy and quality of life, and potentially in economic terms for society and health-care payers. To address the growing impact of type 2 diabetes the International Diabetes Federation (IDF) Taskforce on Prevention and Epidemiology convened a consensus workshop in 2006. The primary goal of the study and this document was the prevention of type 2 diabetes in the commune. A second aim was to reduce the risk of DM disease in people who are identified as being at a higher risk of type 2 diabetes as pre-DM. The study plan for prevention of type 2 diabetes is based on controlling modifiable risk factors and can be focused at pre-DM group

Objectives: 1) To evaluate the blood chemical indicators (glucose, lipid), nutritional status and KAP about DM control and prevention in 3 ward, Thanh Hoa City. 2)To find-out the counseling effect of dietary, food with isomalt supplementation and physical activity in people with pre-diabetes mellitus at commune.
Methods: a intervention control study including 2 phases: 1)
A cross-sectional study carried out in 3 communes (Ngoc Trao,Phu Son & Ba Dinh) including nearly n= 1334 with man=419 and women = 916 from 30-64 years old, of which type 2 DM: 4% and pre-DM: 7,8%, BMI> 23:33,1%.( Oral glucose tolerance test is performed in this screening test). The indicators risk of DM used to assess as nutritional, physical inactivity, obesity is recorded. 2) The Intervention period: in two commune (Intervention Group), the activities of participatory communication on food variety and physical activity was carried out for target subjects during 4 months. In the other commune (Control Group) there was no intervention. Communication materials on negative effects of DM and it’s complications; how to prevent DM and pre-DM;  guidelines on how to develop  food practices and food with low glucose index (Netsue-light, Hura-light with isomalt) were delivered through the network of nutrition collaborators, village health workers, members of  women and farmer association, commune leaders. Glucose blood, BMI, dietary is monitoring regularly in each month

Results: In pre-diabetes people, the prevalence of impaired fasting glucose in interventional group decreased by 65,3% in 4 months (p<0.01).Cholesterol, LDL level in interventional group declined (p<0.01) but increased in conventional group

Subjects with BMI>23 in interventional group decreased more than in conventional group, from 61.5% (n=32) to 28.8% (n=15), p<0.05. After intervention, the consumption of rice and oil per person decreased at a statistical rate (p<0.01). The consumption of vegetables, nuts, fruits increased much in interventional group while meats decreased (p<0.05). Other protein foods like fish, sea foods increased in this group (p<0.01).

The knowledge and practice on diabetes prevention of pre-diabetes in interventional group showed much improvement compared to conventional group. The percentage of type 2 DM progressing is lower in interventional group (13,6%) than control group (27,9%). The consumption of vegetables, nuts, fruits increased much in interventional group while meats decreased (p<0.05). Other protein foods like fish, sea foods increased in this group (p<0.01).

Conclusions: Conselling communication on diatery, food with GI and physical activity was effective to improve people with pre-DM, reducing lipid concentration and BMI. Improving knowledge and practices on DM control and prevention in commune.
